Job summary

Support, Inc. is seeking a Full-Time Exempt Residential Operations role in Colorado to manage a caseload of individuals with developmental disabilities and coordinate health and habilitation services.

The position works within an interdisciplinary team, with duties spanning monitoring, coordination, and ensuring a high quality of life in community settings. The role requires an Associate's Degree or equivalent and at least 2 years of related experience.

Qualifications

• Associate's Degree or equivalent is required.
• At least 2 years of related experience and/or training.

Responsibilities

• Manages a caseload of consumers with intellectual and developmental disabilities who live in apartments
• Oversees all aspects of the consumer's care and ensures each experiences a high quality of life based on their abilities
• Carries out monitoring and quality assurance responsibilities in accordance with the organization's policies and procedures, personnel manual, and applicable laws and regulations
• Facilitates effective relationships between community care providers/team with timely planning, coordination, and conflict resolution, as needed
• Transports and accompanies consumers for grocery shopping, medical appointments, and/or other necessary appointments/tasks.
• Assists in coordinating a variety of recreational activities to enhance social development and meaningful community integration
• Attends site visits on a weekly/bi-weekly basis overseeing the priority needs of consumers
