Qualifications

  • Valid New York State driver’s license and ability to travel between sites and residences.
  • Flexible schedule including evenings, weekends, holidays, or other hours.

Responsibilities

  • Provide individualized case management to residents in the Apartment Treatment Program.
  • Coordinate services within the agency, funding issues, and Medicaid requirements.
  • Maintain and review medication records; monitor weekly logs and counts.
  • Oversee resident medication self-administration following policies.
  • Assist with budgeting and disbursement of funds.
  • Conduct apartment safety checks and inspections for hazards.
  • Coordinate housing-related maintenance with landlords and staff.
  • Support residents in maintaining safe, clean apartments and community living.
  • Conduct monthly fire drills and weekly smoke alarm checks.
  • Travel to residents’ appointments and community locations as needed.

Your role at BHSN:
As an Apartment Program Case Manager, you will empower individuals in our OMH-licensed Apartment Treatment Program to develop the skills, confidence, and community connections needed to live more independently. Through individualized case management, advocacy, medication support, crisis intervention, and hands-on daily living guidance, you will help residents maintain safe and stable homes while making meaningful progress toward their personal goals.

  • Providesaccuraterecordkeeping and client advocacy which includes coordination of services within agency,i.e.funding issues, medication programming, daily living skills training, progress notes, goalplanningand providing requirements for Medicaid billing
  • Maintains medication records for individualclientson a weekly basis andmonitorall incoming medications by completion ofappropriate logsand daily controlled substances counts
  • Overseesresident’smedication self-administration, following proper medication policy/training and protocol
  • Oversees budgeting services and provides funds to resident week/bi-weekly/monthly as agreed upon
  • Observespossible malfunctionsof equipment and contents of apartment; takesappropriate actionwhen needed
  • Completes apartment searches for contraband/weapons when necessary to ensure safety/habilityof the unit
  • Acts as a liaison between client and landlord to resolve maintenance issues quickly
  • Assistsresidents inmaintainingapartments in a safe and sanitary condition
  • Conduct a fire drill on every resident once a month and check smoke alarms on a weekly basis
  • Complete inspections of units to ensure units are safe and complete documentationrequired
  • Manages a resident caseload to include all areas of case management, including effectively coordinating/communicating needs to treatment team members including funding agencies, providers, etc.
  • Makes daily/weekly visits to apartments on an as needed basis
  • Assistrecipients with transition into/out of program
  • Provides ongoing support and referrals to community services as needed
  • Respondsimmediatelyto crisis situations and offer intervention support in conjunction with BHSN Crisis and/or treatment team providers
  • Adjusts schedule to meet the needs of the program and ensure support services are provided
  • Transports residents to and from doctor appointments, grocery shopping, etc., while teachingresidents onhowtoacquireindependent transportation when possible
  • Complies withall BHSN and regulatory policies and procedures

Skills & Qualifications:

  • Associate degree in human services, behavioral health, health care, or a related field and at least three years of relevant experience; or a bachelor’s degree in a related field and at least one year of relevant experience
  • Experience working in mental health, behavioral health, residential services, or a related human services setting
  • Strong interpersonal, communication, organizational, documentation, and time-management skills
  • Ability to manage multiple priorities, respond appropriately to crisis situations, and work both independently and collaboratively
  • Valid New York State driver’s license and ability to travel between program sites, residents’ apartments, appointments, and other community locations as required
  • Reliable transportation consistent with the travel requirements of the position
  • Ability to navigate stairs and assist with lifting or moving residents’ belongings, with or without reasonable accommodation
  • Ability to work a flexible schedule that may include evenings, weekends, holidays, or other nontraditional hours based on program and resident needs
  • Ability to complete all required orientation, medication-related training, and at least five hours of annual professional or educational development
  • Ability to perform the essential functions of the position, with or without reasonable accommodation
