Qualifications

• Graduate of a state-approved nursing program.
• Current, unrestricted nursing license in Massachusetts.
• At least 5 years of experience in a clinical pharmacy setting.
• Experience with EMR systems or related technology.

Responsibilities

• Build therapeutic and supportive relationships with patients and families, providing education and managing care transitions.
• Assess patient symptoms and concerns received via calls, patient portal messages, secure chat, and referrals.
• Prioritize patient needs based on clinical urgency and protocols.
• Escalate complex or urgent issues to GI physicians or APCs.
• Review patient inquiries regarding GI conditions, medications, test results, and treatment plans.
• Provide clinical guidance within nursing scope of practice.
• Contribute to quality improvement and mentor nursing staff, ensuring patient safety.
• Manage EMR in-basket workflows and address portal messages, refill requests, and test results.
• Identify messages requiring provider review and route appropriately.
