Job summary

Baystate Health in western Massachusetts seeks a full-time Ambulatory Nurse Professional for the Transplant Program. The role coordinates evaluation, education, and longitudinal care for transplant candidates, donors, and recipients in inpatient and outpatient settings.

The transplant nurse works with a multidisciplinary team to plan care, manage transitions of care, provide telephone triage, and support program development and regulatory compliance in a patient-centered environment.

Qualifications

• Minimum of 3 years of nursing experience.
• Strong clinical assessment, critical thinking, and care coordination skills.
• Experience in renal, dialysis, and/or transplant care preferred.
• BSN preferred.

Responsibilities

• Coordinate evaluation, work-up, and listing processes for transplant candidates and living donors; manage longitudinal follow-up for post-transplant patients.
• Utilize the nursing process to assess, plan, implement, and evaluate individualized patient care across the continuum.
• Collaborate with physician leaders and interdisciplinary teams to design and improve care delivery systems.
• Demonstrate advanced knowledge of kidney function, CKD, and ESRD treatment options.
• Facilitate multidisciplinary rounds, patient care conferences, and transplant committee presentations.
• Coordinate inpatient admissions, transitions of care, and discharge planning to optimize patient flow.
• Provide comprehensive patient, family, and donor education; deliver telephone triage and clinical guidance.
• Participate in QA and regulatory compliance initiatives; serve as a clinical resource.
