The role involves developing policies, coordinating multi-disciplinary teams, and managing insurance prior authorizations and financial assistance workflows.

Qualifikationen

• Must have Doctor of Pharmacy degree (or foreign equivalent) and minimum one year of pharmacist experience in a hospital-based ambulatory/outpatient or transitions-of-care setting.
• Experience coordinating multidisciplinary teams including nursing, case management, hospitalists, and care transitions teams.
• Experience coordinating transitions-of-care services, including medication reconciliation, discharge counseling, and high-risk medication review.
• Experience developing staff training, competencies, or policy/procedure documents.
• Experience managing insurance prior authorization / financial assistance workflows for discharge medications.
• Must be able and willing to work days, evenings, weekends, and holidays, and be on call on an as-needed basis
• Must have (or be eligible for) a VT pharmacist license.

Aufgaben

• Oversee daily operational workflow of the Meds to Beds program and transitions-of-care services.
• Develop and implement operational policies to identify medication access barriers and liaise with care teams.
• Oversee clinical triage and resolution of ambulatory prescription concerns for retail and Meds to Beds.
• Collaborate with outpatient pharmacy management to develop wraparound services.
