Job summary

The University of Vermont Medical Center in Burlington, VT seeks a Transitions of Care Ambulatory Pharmacist Supervisor to oversee the daily workflow of the Meds to Beds program and the transitions-of-care staff.

You will develop operational policies to address medication access, coordinate with care teams, supervise clinical triage for ambulatory prescriptions, and partner with outpatient leadership to implement wraparound services.

Qualifications

• Doctor of Pharmacy degree (or foreign equivalent).
• Minimum 1 year of pharmacist experience in hospital-based ambulatory/outpatient or transitions-of-care setting.
• Experience coordinating multidisciplinary teams (nursing, case management, hospitalists, care transitions).
• Experience coordinating transitions-of-care services (med reconciliation, discharge counseling, high-risk meds).
• Experience developing staff training, competencies, or policy/procedure documents.
• Experience managing insurance prior authorization/financial assistance workflows for discharge meds.

Responsibilities

• Oversee daily operational workflow of the Meds to Beds program and transitions-of-care services.
• Develop and implement operational policies to identify medication access barriers and liaison with care teams.
• Implement Meds to Beds across units and ensure program success and efficiency.
• Oversee clinical triage and resolution of ambulatory prescription concerns for retail and Meds to Beds.
• Collaborate with Outpatient Pharmacy leadership to develop wraparound services.
