You will coordinate inpatient-to-outpatient discharge processes, oversee clinical triage of ambulatory prescriptions, and partner with nursing and care teams to improve medication access and reduce readmissions.

Qualifikationen

• Must have Doctor of Pharmacy degree.
• Minimum one year of pharmacist experience in a hospital-based ambulatory/outpatient or transitions-of-care setting.
• Must be eligible for VT pharmacist license.

Aufgaben

• Oversee daily operational workflow of the Meds to Beds program and transitions of care staff.
• Coordinate clinical triage and resolution of ambulatory prescription concerns for retail and Meds to Beds operations.
• Develop and implement wraparound services with the Manager, Outpatient Pharmacy.
• Coordinate transitions-of-care services including medication reconciliation and discharge counseling.
