Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• Licensed as LCSW in the state of North Carolina; dual licensure with LCAS preferred.
• Three (3) years of broad clinical experience in healthcare settings for patients with complex medical conditions.

Responsibilities

• Conduct biopsychosocial assessments to evaluate patient needs and barriers.
• Develop individualized care plans integrating medical, psychological, and social aspects of care.
• Connect patients and families with community resources and coordinate referrals.
• Provide patient education, referrals, and transitions of care (home health, hospice, DME, long-term care).
• Document and bill for services in compliance with regulations.
• Identify and manage crises with brief interventions as appropriate.
• Monitor progress and adjust care plans in collaboration with the interdisciplinary team.
• Advocate for patients within the healthcare system and community.
• Practice active listening, empathy, and culturally sensitive care.
• Utilize motivational interviewing to support behavior change.
