Qualifications

• Three years of broad clinical healthcare experience.

Responsibilities

• Conduct biopsychosocial assessments and develop patient-centered care plans.
• Coordinate resources, referrals, and transitions of care.
• Advocate for patients and families within the healthcare system.
• Provide crisis intervention and ongoing psychosocial support.
• Document services and ensure billing compliant with regulations.
• Collaborate with multidisciplinary teams and participate in quality initiatives.
