Jefferson Health is seeking a Registered Nurse to support the spine and brain metastasis program growth in ambulatory clinics. The nurse will collaborate with providers, deliver evidence-based nursing care, and educate patients and families to meet physical, emotional, and social needs.

Competenze

• Specialized Diploma graduate in nursing required.
• BSN or completion of BSN or appropriate advanced degree within 3 years of hire strongly preferred.
• Current nursing license in the state of practice is required.
• BLS (AHA) certification is required.
• 1 year experience as a nurse in an ambulatory setting is preferred.

• Provides patient and family education for preventive care, chronic disease management, and medication management in the primary care setting.
• Documents patient interactions and nursing care in EMR according to organizational standards and regulations.
• Performs care coordination for patients (abnormal results, triage, vaccine verification, and related forms).
• Oversees clinical care essential to health promotion and restoration.
• Minimizes risk of harm by following established nursing practices and protocols.
• Uses information technology to communicate, manage knowledge, and mitigate errors.
• Uses data to monitor care outcomes and supports ethical decision making; collaborates to improve quality and safety.
• Works within ambulatory care inter-professional teams fostering open communication and shared decision-making.
