Job summary

Foundation Medical Partners, affiliated with Southern New Hampshire Medical Center, seeks a qualified RN to assist providers, admit patients, take vitals, triage, and manage infusion therapies including biologics, with careful documentation in the EHR.

Work Monday–Friday 8:00–5:00 with flexible day hours and no weekends; collaborate with Physicians and PAs, perform point‑of‑care testing, administer vaccines, and educate patients on self‑management.

Qualifications

• Current NH RN licensure is required.
• Nursing degree is required.
• BLS/CPR certification required.
• Epic experience is a plus; IV experience preferred.

Responsibilities

• Carry out the nursing process in person, by telephone, and via telehealth modalities.
• Assess patient needs, plan care with the interdisciplinary team, and implement the plan of care.
• Administer prescribed medications, vaccines, and treatments; perform point-of-care testing and collect specimens.
• Educate patients on self-management activities and health maintenance.
