Job summary

Qualifications

• RN license in state of practice required.

• Experience in chronic disease management and familiarity with disease terminology.

Responsibilities

• Identify patients for proactive interventions using screenings and referrals.
• Develop and monitor individualized care plans aligned with NCQA guidelines.
• Educate patients and families to support self-management and behavior change.
• Collaborate with physicians and care teams to coordinate services and transitions of care.
• Assess social determinants of health and connect patients with community resources.
• Participate in data analysis to improve outcomes and reduce unnecessary utilization.
