Youll coordinate and provide care including wound management, injections, procedures, surgical and post-operative education, and ongoing patient supportall while helping patients feel informed, confident, and cared for every step of the way.

The Ambulatory RN Care Coordinator utilizes a collaborative process of assessment, planning, implementation and evaluation to engage, educate, promote and maintain optimal health status of assigned patients. This position functions in a collaborative partnership with the patient, physician and the care team in the office setting. This position serves in an advocacy role to ensure the delivery of quality, efficient, and cost-effective patient-centric health care services. The Ambulatory RN Care Coordinators case load consists of moderate to high risk patients.
