Lee Health in Estero, FL is seeking an experienced Ambulatory RN Care Coordinator to join the oncology team. You will coordinate oncology patient care, serve as a resource for patients and families, and provide telephone triage and follow-up coordination in a collaborative office setting.

The role requires at least 3 years of clinical nursing experience, Florida RN licensure, and a focus on patient education and care coordination within a high-risk patient load.

Qualifikationen

Experience in oncology or care coordination is preferred.

What will this RN do?

Coordinate care for oncology patients across the continuum of care.
Serve as a clinical resource and advocate for patients and families.
Provide telephone triage and respond to patient questions, concerns, and clinical needs.
Manage and respond to in-basket messages and coordinate appropriate follow-up.

The Ambulatory RN Care Coordinator utilizes a collaborative process of assessment, planning, implementation and evaluation to engage, educate, promote and maintain optimal health status of assigned patients. This position functions in a collaborative partnership with the patient, physician and the care team in the office setting. This position serves in an advocacy role to ensure the delivery of quality, efficient, and cost-effective patient-centric health care services. The Ambulatory RN Care Coordinators case load consists of moderate to high risk patients.
