Job summary

Qualifications

• High school diploma or equivalent required.
• Healthcare administration, health information management, public health or related field degree preferred.
• Experience with electronic medical records preferred.
• Basic medical terminology knowledge preferred.
• Strong interpersonal and telephone communication skills.
• Proficient computer, documentation, and data-entry skills.
• Ability to review high volumes of information with accuracy.
• Ability to handle confidential patient information.

Responsibilities

• Identify open care gaps using quality reports and patient outreach.
• Contact patients when outreach is needed and retrieve supporting records from approved sources.
• Document outreach and findings in Epic and complete care gap workflows when requirements are met.
• Collaborate with the ambulatory care team to improve documentation and care quality goals.
