Community Health Network is looking for an Ambulatory Triage Registered Nurse located in Indianapolis, Indiana. The role involves phone triage, patient education, appointment scheduling, and care coordination within the Pulmonary Critical Care & Sleep Medicine team.

The ideal candidate must be a graduate from an accredited nursing program, possess a valid RN license in Indiana, and have at least one year of nursing experience. Exceptional communication and team skills are essential for success.

Qualifications

• Graduate of NLN, CCNE, ACEN, or NLN CNEA accredited nursing school, or three years of related professional nursing experience.
• Licensed as a Registered Nurse (RN) with a valid Indiana license.
• One year or more of nursing experience required.
• Ambulatory experience highly preferred.
• Experience in pulmonary critical care, sleep medicine, or ICU preferred.
• Phone triage experience a plus.

Responsibilities

• Conduct phone triage and provide patient education.
• Schedule appointments and manage prescription refills.
• Coordinate care and perform limited direct patient care as needed.
