Job summary

The Health Federation of Philadelphia is seeking a Physical Therapist to join the Ambulatory Health Services team in a municipal health clinic in Philadelphia. The ideal candidate will assess and treat musculoskeletal and neuromuscular conditions, providing evidence-based, patient-centered care.

You will work in an integrated care capacity, collaborating with medical and behavioral health colleagues, supporting diverse populations, and promoting self-management and functional improvement.

Qualifications

• DPT from a CAPTE-accredited program.
• Licensed PT in Pennsylvania.
• Direct Access Certification within 6 months of hire.

Responsibilities

• Assess, diagnose, and treat musculoskeletal and neuromuscular conditions in primary care.
• Provide therapy plans and education for self-management.
• Collaborate with medical, behavioral health, and support staff.
• Ensure patient safety and engagement in care.
• Participate in interprofessional coordination and communication.
