Job summary

Rush University Medical Center in Chicago seeks an Ambulatory Pharmacy Technician – Transition of Care Specialist to join the Pharmacotherapy Clinic. The role collaborates with ambulatory clinical pharmacy specialists to improve patient care through coordination, scheduling and medication access support.

You will work on care coordination, communicate with patients and caregivers, perform medication reconciliations and help ensure high-quality service within a hospital ambulatory setting.

Qualifications

• Requires HS diploma or equivalent with healthcare experience.
• Five years in an alternate site care environment preferred.
• Strong problem solving, communication and organizational skills.
• Experience delivering customer service to internal and external customers.
• Knowledge of regulations, policies and Medicare/insurance basics.
• Proficiency with Word, Excel and clinical databases.

Responsibilities

• Manage caseload and set priorities within ambulatory care.
• Collaborate with clinical pharmacy team to coordinate care.
• Support scheduling, patient communication and care coordination.
• Document activities in clinical software and generate reports.
• Provide training and share expertise with team members.
• Promote continuous process improvement and problem solving.
