Job summary

UF Health Jacksonville is seeking an Ambulatory Pharmacist Practitioner to deliver comprehensive pharmaceutical care across ambulatory and specialty services, collaborating with providers, nurses, and staff.

You will perform MTM and medication reviews, reconcile therapies, provide patient education and adherence support, handle prior authorizations, access coordination, and oversee prescription fulfillment and delivery operations.

Qualifications

Bachelor’s degree in Pharmacy or PharmD required
Florida pharmacist license or eligibility within 90 days of hire/transfer
Less than one year pharmacist/intern experience
Post-graduate residency preferred
Attention to detail, fine motor coordination, and calculations
Excellent communication and customer service skills
Supervision of technician employees (3-18 staff)

Responsibilities

Provide Medication Therapy Management (MTM) and comprehensive medication reviews
Conduct medication reconciliation and resolve medication-related problems
Provide patient education and adherence counseling
Monitor therapeutic outcomes and support optimization efforts
Assist with transitions of care and outpatient medication management
Facilitate prior authorizations, appeals, formulary exceptions, and payer documentation
Coordinate patient assistance programs and financial support resources
Oversee prescription fulfillment, shipping, and medication delivery operations
