This full-time position offers relocation incentives up to $7,500, opportunities for medication use evaluations, patient education, and comprehensive medication management via in-person or virtual visits.

Qualifications

• PharmD required or BS Pharmacy degree with acceptable experience.
• Florida Pharmacist Licensure required.
• BCACP preferred.
• Five years of pharmacist experience required for BS degree holders.
• Ambulatory experience or PGY-1 preferred for PharmD candidates.

Responsibilities

• Integrated into primary care to improve patient outcomes with evidence-based prescribing.
• Conduct medication use evaluations and support patient access and adherence.
• Leverage analytics to identify patients who benefit from interventions and collaborate with providers.
• Perform detailed medication reconciliation.

Minimum to Midpoint Pay Rate: $55.50 - $67.01 / hour

• The LPG Pharmacist will be integrated into primary care with a focus on improving patient outcomes through assuring optimal prescribing practices consistent with evidence-based literature and treatment guidelines.
• Conduct medication use evaluations, and support patient access and adherence to therapy.
• Utilize and leverage analytic data to determine which patients will benefit from interventions, medication education and collaborate with primary care providers and specialists in the care of these patients.
• Perform detailed medication reconciliation.
• Conduct virtual or face to face visits to provide comprehensive medication management.
• Provide recommendations to providers regarding utilization of high-cost medications.
• Participate in the development of quality measures and quality improvement initiatives.
• Provide educational presentations to providers.
• Provide technician supervision to ensure accurate and timely submissions of prior authorizations.

Strong communication and interpersonal skills, analytical skills, and leadership skills. Collaborates well in a team environment. Uses effective conflict resolution skills. Proficient in use of the EMR. Willing to serve as a preceptor to pharmacy students and residents.

Certification requirements Preferred:

Ambulatory Care Pharmacy Specialty Certification (BCACP)

Minimum years of experience Preferred:

3 years of inpatient or ambulatory pharmacy experience, or completion of PGY 1

Requirements

Education: PharmD required or BS Pharmacy degree with equivalent and acceptable experience from a College of Pharmacy accredited by American Association of Colleges of Pharmacy (AACP).

Experience: Five years of professionally responsible pharmacist experience required for Bachelors degree holders. One year of recent ambulatory experience as a pharmacist or pharmacy intern for PharmD candidates.

Certification: BLS (American Heart Association/Basic Life Support) required (must be obtained within first 90 days of hire).

License: State of Florida Department of Professional Regulation, Registered Pharmacist Licensure.

Other: Strong communication and interpersonal skills, data entry skills, analytical and math skills required.
