Job summary

Lee Health in Fort Myers, FL is hiring an Ambulatory RN Care Coordinator to support oncology patients through their cancer journey in an outpatient setting, coordinating care and education with families and providers.

The RN will assess, plan, implement and evaluate care, work closely with physicians and the care team, manage a moderate to high risk caseload, and strive for quality, efficient, patient-centric service.

Qualifications

• Associate's Degree in Nursing is required.
• Bachelor's Degree in Nursing is preferred.
• Minimum 3 years of clinical nursing experience with emphasis on patient education and chronic disease management.

Responsibilities

• Coordinate care for ambulatory oncology patients in an outpatient setting.
• Educate patients and families, promoting health and wellness.
• Collaborate with physicians and multidisciplinary team to ensure high-quality, cost-effective care.
• Manage a moderate to high risk patient caseload.
