Job summary

The role supports providers, delivers direct patient care in the clinic, and ensures high-quality, cost-effective care with a focus on patient education and adherence to clinical protocols.

Ideal candidates have 2–5 years of nursing experience, an RN license with BLS, and nursing education as a Diploma/Associate.

Qualifications

• Direct patient care in ambulatory clinic settings.
• Collaboration with providers and care team to develop plans of care.
• Documentation of patient assessments and treatments per SOPs.

Responsibilities

• Collaborate with PCPs and Specialists to develop and implement care plans.
• Assess patients’ physical, psychosocial, and emotional needs.
• Document patient assessment and treatment per compliance standards.
• Provide patient education and support using appropriate tools.
• Support providers in daily patient management in a clinic setting.
