Short Description

Responsible for the facilitation and coordination of safe and comprehensive care to all patients presenting to Multi‑Specialty Site. This Ambulatory Nurse coordinator is a professional caregiver who, as a member of a multidisciplinary health care team, assumes responsibility and accountability for a group of patients for a designated time frame and who provides care to these patients utilizing the nursing process including the therapeutic use of self. Leadership may be remote; requires ability of expanded problem‑solving, critical thinking and decision‑making at a supervisory level. Will have the unique opportunity to assist in the development and operations of the Cooper Moorestown Multidisciplinary site. This model focuses on the acute/immediate care needs in the Moorestown and surrounding areas. Excellent customer service, interpersonal and clinical skills are required.

The coordinator works closely with multiple providers and ancillary team members in an ambulatory setting. Providing direct patient care, telephone triage, performing prior authorizations, and other duties related to patient care as directed from provider and or direct supervisor/manager.

Experience Required

1 year RN in Ambulatory setting preferred

Education Requirements

Graduate of NLN accredited School of Nursing, BSN preferred.

License/Certification Requirements

Current NJ-RN License (Registered Nurse)

Current NJ-BLS Certification (Basic Life Support)

Special Requirements

The Ambulatory Nurse Coordinator requires critical thinking to consider factors related to patient safety, effectiveness, and cost in delivering care. Effective resource utilization by the Nurse Coordinator requires coordination with the Clinical Director to plan or change course of action for improved continuity of care and outcomes. Requires expanded problem‑solving, critical thinking and decision‑making at a supervisory level, including excellent communication skills and the ability to delegate effectively and supervise the work of other team members.
