Job summary

UNC Health is seeking a Clinical Pharmacist Practitioner to join the Nephrology Clinic in Chapel Hill, serving as an Advanced Practice Provider within an inter-disciplinary team. The role focuses on CKD management, guideline-directed therapy, and complex nephrology medications in a hybrid clinic.

The CPP will mentor pharmacy students and residents, contribute to quality improvement, and participate in educational activities with UNC Eshelman School of Pharmacy.

Qualifications

• Completion of PGY2 specialty residency in Ambulatory Care or Pharmacotherapy preferred.
• 3–5 years of ambulatory care experience preferred.
• Board Certification in Ambulatory Care or Pharmacotherapy preferred.

Responsibilities

• Build a clinical practice with physicians to care for clinic patients and determine treatment plans.
• Provide longitudinal dosing and monitoring, including lab monitoring and medication management.
• Educate patients, families, and staff on treatment plans and guideline updates.
• Lead quality improvement initiatives and participate in teaching and research activities.
