Job summary

Your responsibilities include patient assessments, education, lab monitoring, and documentation in the EHR, with opportunities to contribute to quality improvement and teach students.

Qualifications

• Pharmacotherapy knowledge for chronic/complex conditions.
• Understanding of CA pharmacy regs and CPA.
• Familiarity with EHR documentation and outcomes tracking.
• Ability to assess patients and develop treatment plans under CPA.
• Patient education and counseling incl. telehealth.
• Proficiency with Epic or equivalent EHR systems.

Responsibilities

• Provide MTM services in HOPD pharmacist-managed clinics.
• Initiate/modify/discontinue medication therapy under CPA.
• Conduct patient assessments and med reconciliation.
• Order and monitor labs for therapy efficacy/safety.
• Manage high-risk chronic diseases under CPA.
• Develop evidence-based treatment plans.
• Document encounters in EHR and track outcomes.
• Precept pharmacy students and residents.
