Qualifications

• Current California license as LCSW.
• 3+ years of work experience in a healthcare setting.
• Certification: Advanced Certified Hospice and Palliative Social Worker or to obtain within 3 years.
• CPR certification current.
• Experience with regulatory requirements related to reporting of child/elder abuse, domestic violence, and other legal requirements.

Responsibilities

• Assess and intervene with patients and family to identify psychosocial factors.
• Develop and implement care plans with interdisciplinary team.
• Coordinate with community resources and referrals.
• Provide psycho-social therapy using brief and crisis methods.
• Assist in transitional and discharge planning and staff education.
