Job summary

Bayhealth Medical Center in Delaware is seeking an Ambulatory Clinical Pharmacy Specialist to provide ambulatory clinical pharmacy services and to operate as a credentialed, privileged member of the healthcare team under collaborative practice agreements (CPAs) to optimize therapy, access, and costs.

You will partner with prescribers, nurses, and multidisciplinary teams to design, monitor, and document plans reflecting safe, evidence-based medication use, while supporting chronic disease care,

Qualifications

• Pharmacist with a Bachelor degree and licensed in the state of Delaware.
• BPS certification in Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics.
• At least one year of clinical pharmacy experience, including residency.
• Ability to collaborate with care teams and provide patient counseling.

Responsibilities

• Provide ambulatory clinical pharmacy services under a pharmacy CPA as credentialed by Bayhealth MEC.
• Conduct patient visits for medication management, including reconciliation, monitoring, counseling, and disease state management.
• Collaborate with providers to implement evidence-based interventions that improve quality and reduce costs.
• Support patient access through prior authorizations and financial assistance programs.
• Oversee specialty medications for safety and continuity of therapy.
• Provide drug information and education to patients and staff.
• Precept students, interns, or residents as assigned.
