Qualifications

• Bachelor of Science in Pharmacy required.
• Current Pharmacy license in the state of practice.
• Preferred Doctor of Pharmacy (PharmD).
• Completion of PGY1 Pharmacy Residency; PGY2 optional.
• Strong computer and data-entry skills; proficient with Windows.

Responsibilities

• Provides pharmacokinetic consultation and uses laboratory data to evaluate drug dosing.
• Evaluates patient response to drug therapy.
• Documents interventions and conducts outcome evaluations.
• Assists development of pathways, best practices, and guidelines.
• Educates staff, students, patients and health care providers; mentors residents.
