Job summary

Ochsner Health is seeking a Pharmacy professional responsible for medication management, pharmacokinetic consultation, and education across patient care transitions. This role involves evaluating therapy, mentoring learners, and developing practice guidelines within a collaborative health system.

The position emphasizes leadership, teaching, and adapting communication to diverse patient populations while ensuring privacy and compliance with regulatory standards.

Qualifications

• Must have a current Pharmacy license in the state of practice.
• Completion of PGY1 Residency (or 3 years related specialty accepted).
• Preferred Doctor of Pharmacy (PharmD).
• Completion of a PGY2 Pharmacy Residency.

Responsibilities

• Provides pharmacokinetic consultation, obtains and uses appropriate laboratory data to evaluate drug therapy and dosing; communicates findings to health care practitioner.
• Evaluates patient response to drug therapy.
• Documents interventions and conducts outcome studies/evaluations of clinical activities.
• Assists with development of pathways, best practices, and guidelines for the assigned population.
• Educates staff, students, patients and health care providers; submits for publication once every two years.
• Mentors residents, students and new staff to meet departmental goals.
• Responsible for overall patient care beyond the hospital visit, through all transitions of care.
• Volunteers to perform extra work during staffing shortages due to inclement weather, outages, or other situations.
• Adapts communication to the patient population, respecting privacy and consent.
• Other related duties as required.
