Job summary

Mercy Springfield is seeking a Clinical Pharmacy Specialist - Ambulatory Care to optimize medication therapy in an ambulatory setting. You will collaborate with physicians, nurses, and care teams to improve patient outcomes and provide expert drug information to staff and patients.

The role emphasizes top-of-license practice, education, quality initiatives, and active participation in scholarly activities within ambulatory care.

Qualifications

• Doctor of Pharmacy (PharmD) or BS in Pharmacy from an accredited School of Pharmacy.
• Current Pharmacist license in the state of practice.
• Missouri pharmacists must obtain Medication Therapy Services Certificate within 90 days of hire.
• Minimum of 5 years of active clinical practice in a specialty area.
• Ambulatory Care experience strongly preferred.
• Active Board of Pharmacy Specialties (BPS) Certification preferred, ideally within specialty practice area.

Responsibilities

• Review patient records and evaluate medication therapy appropriateness.
• Develop and implement individualized medication treatment plans.
• Identify, prevent, and resolve medication-related problems.
• Monitor patient outcomes and adjust pharmacotherapy recommendations as needed.
• Conduct comprehensive medication assessments and Medication Therapy Services.
• Collaborate with physicians and healthcare providers on drug selection, dosing, administration, contraindications, and drug interactions.
• Provide medication counseling and education to patients and caregivers.
• Serve as a clinical resource and subject matter expert within ambulatory care practice areas.
