Qualifications

• Pharmacist degree or PharmD from an accredited school.
• Washington Pharmacist License upon hire.
• Board Certification or accredited residency preferred.

Responsibilities

• Assess patient-specific medications and clinical data to evaluate safety, efficacy and pharmaco-economic optimization opportunities.
• Administer medications as needed and monitor clinical data.
• Educate patients and provide drug information to the healthcare team.
