Job summary

Aspirus Merrill Clinic in Merrill, WI seeks an RN Ambulatory Care Coordinator to assess, coordinate, and integrate care for chronic conditions across the clinic system. You will collaborate with physicians and health professionals to deliver patient-centered care and population health initiatives.

The role features full-time hours (80 per pay period) with 8-hour day shifts, Monday through Friday, within Aspirus’s care continuum. RN license, BLS, ACLS, and BSN are required or in progress as noted.

Qualifications

• Active Wisconsin RN license is required.
• Current BLS certification required.
• ACLS certification required for adult units; NRP for NICU.
• BSN degree required or actively pursuing within 4 years of hire.
• Masters in Nursing preferred; leadership certifications encouraged.

Responsibilities

• Assess, coordinate, and integrate care for patients with chronic conditions.
• Collaborate with physicians, staff, and other health professionals to deliver care across the clinic.
• Provide population health management practices within Aspirus Clinic System.
