Qualifications

• Specialized Diploma Graduate of an accredited school of nursing.
• Knowledge of patient-and family-centered care and effective collaboration with designated care partners.
• HIPAA confidentiality and professional nursing standards.

Responsibilities

• Integrates evidence-based practice with clinical expertise for delivery of optimal healthcare.
• Collaborates with clinic care providers to support timely communication between primary care and specialists.
• Supports implementation of clinical care pathways and participates in quality improvement opportunities.
• Utilizes EMR and nursing interventions during visits, telephone encounters, and electronic communication.
• Educates patients and families on preventive care, chronic disease management, medications, and vaccinations.
