Jefferson Health ambulatory care clinic is seeking an experienced RN to provide comprehensive nursing care within a multidisciplinary team at Albert Einstein Medical Center’s East Norriton location. You will apply the nursing process to address physical, psychological, and social needs, while maintaining HIPAA-compliant documentation and fostering patient-centered care.

The position requires an active PA RN license, BLS, and strong communication skills to educate patients and coordinate care

Qualifications

• Specialized Diploma Graduate of an accredited school of nursing required.
• Patient- and Family-Centered Practice knowledge and collaboration with care partners.
• Knowledge of diverse backgrounds and cultural sensitivity.
• Effective communication skills for interactions with patients and families.
• Professionalism, confidentiality, HIPAA adherence, and ethical behavior.
• RN license in PA and current practice.
• Knowledge of nursing terminology and patient assessment throughout the care continuum.
• Awareness of basic life support requirements (AHA BLS).

Responsabilités

• Integrates evidence-based practice with clinical expertise and patient preferences to deliver optimal care.
• Collaborates with clinic care providers to ensure timely communication and coordinated care.
• Supports implementation of care pathways and quality improvement activities.
• Uses EMR and nursing care documentation according to standards and regulations.
• Educates patients and families on preventive care, chronic disease management, and medication management.
• Performs care coordination including triage, vaccinations verification, and prior authorizations as needed.
• Contributes to safe, compassionate, and coordinated care within ambulatory teams.
• Participates in team huddles and accepts feedback for continuous improvement.
