Job summary

Aspirus Chippewa Falls Hospital in Chippewa Falls, WI is seeking an RN Ambulatory Care Coordinator to assess, coordinate, and integrate care for patients with chronic conditions within its clinic system.

This full-time role collaborates with physicians and staff to deliver care across the health care continuum and to support population health management for local patients and communities.

Qualifications

• Must hold an active Wisconsin RN license and BLS certification.
• BSN required, or actively pursuing degree with 4-year completion plan.
• Two years of department/specialty experience; supervisory/leadership helpful.
• ACLS and NRP for NICU preferred.
• Professional nursing organization membership required or pursued within 3–5 years.

Responsibilities

• Assess, coordinate, and integrate care for patients with chronic conditions in Aspirus clinics.
• Collaborate with physicians, staff, and other health professionals to provide services across the care continuum.
• Support population health management initiatives and patient education within the ambulatory setting.
