Job summary

Aspirus Chippewa Falls Hospital in Chippewa Falls, WI is seeking an RN Ambulatory Care Coordinator to assess, coordinate, and integrate care for patients with chronic conditions within its clinic system.

This full-time role collaborates with physicians and staff to deliver care across the health care continuum and to support population health management for local patients and communities.

Qualifications

• Must hold an active Wisconsin RN license and BLS certification.
• BSN required, or actively pursuing degree with 4-year completion plan.
• Two years of department/specialty experience; supervisory/leadership helpful.
• ACLS and NRP for NICU preferred.
• Professional nursing organization membership required or pursued within 3–5 years.

RN AMBULATORY CARE COORDINATOR

Aspirus Chippewa Falls Hospital, Chippewa Falls, WI

Full-Time or 1.0 FTE, 80 hours every pay period
8-hourDay shifts, Monday - Friday

The RN Ambulatory Care Coordinator is a Registered Nurse (RN) directly responsible for assessing, coordinating, and integrating healthcare for patients with chronic conditions in the Aspirus Clinic System. This RN position manages all aspects of patient centric care coordination through collaboration with physicians, staff, and other health professionals to provide services across the health care continuum for all patients within the clinic setting. The Ambulatory Care Coordinator will work directly with the entire health care team to deliver innovative population health management practices to the patients and community of Aspirus.

Experience/Qualifications:

• Knowledge of patient care standards and practices is normally acquired through completion of a nursing program.
• Two years' experience in department/specialty necessary to enhance and develop skills, with previous experience in supervisory management, leadership, or teaching capacity beneficial.
• Possesses an active Wisconsin RN nursing license.
• Current BLS certification - All clinical personnel in identified job codes are required to maintain Basic Life Support (BLS) training through independent study, online manikin, and knowledge testing. An acceptable training program is the Basic Life Support (BLS) Health Care Provider course from the American Heart Association or the American Red Cross Professional Rescuer Program with AED. • Advanced Cardiac Life Support (ACLS) for adult patient units and NRP for NICU.
• BSN required, or actively working towards degree to be obtained within 4 years of the hire date in that role.
• Additional specialty certifications as specified by department protocol (i.e. PALS, TNCC, etc.)
• Certification for area of specialty required or required to pursue certification in professional development within three to five years of eligibility.
• Master's in nursing preferred.
• Member of professional nursing organization required (i.e. ENA, CCRN, AACN, etc.)

Aspirus Chippewa Falls Hospital and Clinic is scheduled to open in September of 2026. The newest location in the Aspirus Health system will feature a hospital with 10 inpatient beds, emergency department with helipad to support emergency transport needs, on-site clinic with 12 treatment rooms, imaging services (X-ray and CT), laboratory services, and a pharmacy.
