Job summary

Aspirus Chippewa Falls Hospital in Chippewa Falls, WI, seeks a Registered Nurse to coordinate ambulatory care for patients with chronic conditions within the clinic system. You will assess and integrate care across the health care continuum in collaboration with physicians and the care team.

The role emphasizes population health management, care coordination, and patient-centered services across the Aspirus Clinic System.

Qualifications

• Active Wisconsin RN license required.
• BLS certification required for clinical staff.
• ACLS required for adult units; NRP for NICU as applicable.
• BSN is required or actively pursued within four years of hire.
• Experience in supervision or leadership is beneficial.

Responsibilities

• Assess and coordinate care for patients with chronic conditions in the clinic system.
• Collaborate with physicians, staff, and health professionals to deliver patient-centered care.
• Manage care coordination across the health care continuum within Ambulatory Care.
• Contribute to population health management initiatives within the clinic.
