Responsibilities

• Stabilize care delivery through early risk detection and escalation.

• Translate provider intent into reliable execution across follow-ups and next steps.
• Reduce operational friction by coordinating with MAs, case management, and operations.
• Strengthen patient understanding and agency through clear, respectful communication.
