Job summary

CommonSpirit Health's Mercy Hospital in Durango, CO is seeking a Clinic RN to facilitate collaboration with the healthcare team and patients/families, promoting engagement and self-management. You will use the nursing process to maximize safety and outcomes, adhering to standards and the Nurse Practice Act while upholding patient-centered care.

The role requires strong clinical acumen in ambulatory care, knowledge of community health concepts, and expertise in case management and coordination

Qualifications

• Current RN license in good standing
• BLS through American Heart Association
• Ability to lift up to 50 lbs occasionally, 20 lbs frequently, 10 lbs constantly

Responsibilities

• Assess patient needs through observation, interviews, and review of medical records/usual clinical data.
• Evaluate patient responses and intervene to optimize outcomes.
• Develop and implement patient care plans and observe outcomes alongside the interdisciplinary team.
• Demonstrate competency in therapeutic/diagnostic interventions including IVs, wound care, medication administration, catheterization, and specimen collection.
• Provide education to patients and families and document status, interventions, and outcomes.
