Job summary

Grace Medical Center, part of LifeBridge Health, seeks a Referral Coordinator to manage patient referrals for program services recommended by providers. Under leadership supervision, the role ensures referrals are routed to the appropriate programs and disciplines.

The position works with EMR and other platforms to retrieve referrals, tracks status, conducts outreach, and troubleshoots issues. It also supports KPI tracking and reporting dashboards to leadership.

Qualifications

• Essential: 1-3 years relevant experience

Responsibilities

• Identify and route referrals to Ambulatory Care Management programs.
• Retrieve referrals from EMR, email, phone, risk reports, and other sources.
• Track referral status and perform patient outreach as necessary.
• Troubleshoot referral process issues and escalate to leadership when needed.
• Assist leadership in ensuring program referral KPIs are tracked and reported regularly.
• Ensure KPIs are tracked as part of the program dashboard.
• Engage analytics and reporting teams to develop dashboards and regularly report KPIs to leadership.
• Monitor reports for inaccuracies and engage stakeholders to resolve issues.
