Job summary

UVA Physicians Group in Charlottesville, VA seeks an Ambulatory Care Clinical Pharmacist to deliver evidence-based pharmaceutical care in the ambulatory setting at two primary care locations: UVA Primary Care Riverside (Charlottesville) and Medical Associates of Louisa in Louisa.

The pharmacist designs and monitors therapeutic plans, collaborates with physicians and interprofessional teams, provides patient education, and supports access to affordable care while pursuing professional development

Qualifications

• Pharm.D. or ACPE-accredited degree required.
• PGY-1 residency or equivalent clinical experience.
• Virginia Board of Pharmacy license; AHA BLS; Board certification encouraged.

Responsibilities

• Provide evidence-based ambulatory care to patients.
• Design, implement, monitor, and document therapeutic plans.
• Collaborate with providers and interprofessional teams.
• Support education and professional development.
• Perform other duties as assigned.
