The Pharmacy Department at Hartford Hospital utilizes a decentralized model with pharmacists embedded on medical teams to advice on optimal drug therapy. We also have an American Society of Health-System Pharmacists (ASHP) accredited Pharmacy Residency program, an active Oncology infusion service, and a Retail/Specialty outpatient pharmacy.
The Ambulatory Care Pharmacy Specialist will collaborate as an integral member of the multidisciplinary team in the provision of care for patients in our medical clinics to optimize and promote rational, safe, evidence-based and cost-effective therapy. In addition to direct patient care responsibilities, the clinical pharmacy specialist will: actively participate in committees and /or workgroups; collaborate in the review of new and existing medications, clinical practice guidelines and other related policies used in the management of the clinic-specific medical conditions. This person also conducts clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice.
