Job summary

Salem Health is seeking a pharmacist for their Medication Management clinic in Salem, OR. The successful candidate will collaborate with healthcare teams to optimize patient medication therapy, provide clinical services, and ensure safe and effective medication management.

Applicants must hold a Doctor of Pharmacy degree or a Bachelor's in Pharmacy, possess an Oregon pharmacist license, and be certified as a BCPS within two years of hire.

Qualifications

Doctor of pharmacy degree or a bachelor's degree in pharmacy with relevant experience.
Active State of Oregon pharmacist license.
BCPS certification within two years of hire date.
BLS certification within 90 days of hire.

Responsibilities

Interprets treatment goals and recommends medication orders.
Participates in medication prescribing under Collaborative Practice Agreements.
Provides collaborative drug therapy management services.
