Job summary

IU Health is seeking an Ambulatory Care Pharmacist to provide medication management at McCordsville and Fishers Primary Care clinics. The role emphasizes chronic disease management, patient education, and collaboration with physicians and nurses.

Qualifications

  • Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.
  • Active Indiana pharmacist license or eligibility for licensure.
  • PGY1 ambulatory care experience or equivalent.
  • Strong clinical knowledge in chronic disease management.

Responsibilities

  • Conduct comprehensive medication reviews and assessments for patients.
  • Provide medication therapy management for chronic conditions such as diabetes, hypertension, hyperlipidemia, and heart failure.
  • Initiate, adjust, and monitor therapy under collaborative practice agreements.
  • Educate patients and caregivers on medications and self-management strategies.
  • Document encounters accurately in the electronic medical record and support compliance.

Ambulatory Care Pharmacist
Location:
  • McCordsville Primary Care (2 days per week)
  • Fishers Primary Care, Fishers Hospital Medical Office Building (3 days per week)
Position Summary

The Ambulatory Care Pharmacist is an integral member of the primary care team, providing comprehensive medication management services to optimize patient outcomes. Working collaboratively with physicians, advanced practice providers, nurses, and other healthcare professionals, this role focuses on chronic disease state management, medication optimization, patient education, and population health initiatives. The pharmacist will provide direct patient care in both the McCordsville and Fishers Primary Care clinics under collaborative practice agreements and organizational protocols.

Key Responsibilities
Direct Patient Care
  • Conduct comprehensive medication reviews and assessments.
  • Provide medication therapy management for patients with chronic conditions including, but not limited to:
    • Diabetes
    • Hypertension
    • Hyperlipidemia
    • Heart failure
    • Chronic kidney disease
    • Anticoagulation management
    • Obesity and weight management
  • Initiate, adjust, and monitor medication therapy in accordance with collaborative practice agreements and evidence-based guidelines.
  • Order and interpret laboratory tests as authorized.
  • Educate patients and caregivers regarding medications, disease states, treatment goals, and self-management strategies.
  • Monitor therapeutic outcomes and medication adherence.
Collaborative Care
  • Partner closely with primary care providers to develop and implement individualized treatment plans.
  • Participate in interdisciplinary team meetings and care coordination activities.
  • Provide drug information and medication-related recommendations to healthcare team members.
  • Support transitions of care and medication reconciliation efforts.
Population Health & Quality Improvement
  • Identify and manage high-risk patient populations.
  • Support quality metric improvement initiatives, including HEDIS, STAR, and value-based care measures.
  • Utilize electronic health record and population health tools to identify care gaps and recommend interventions.
  • Contribute to clinic and system-wide quality improvement projects.
Documentation & Compliance
  • Document patient encounters accurately and timely within the electronic medical record.
  • Maintain compliance with state and federal regulations, organizational policies, and collaborative practice agreements.
  • Support departmental reporting and outcomes tracking.
Qualifications
Required
  • Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.
  • Active pharmacist license in the State of Indiana, or eligibility for licensure.
  • Completion of a PGY1 Pharmacy Residency or equivalent ambulatory care experience.
  • Strong clinical knowledge in chronic disease state management.
  • Excellent communication, patient counseling, and interdisciplinary collaboration skills.
Preferred
  • PGY2 Ambulatory Care Residency.
  • Board Certified Ambulatory Care Pharmacist (BCACP) or other applicable board certification.
  • Experience practicing in a primary care or ambulatory care setting.
  • Experience operating under collaborative practice agreements.
Work Schedule
  • 2 days per week: McCordsville Primary Care
  • 3 days per week: Fishers Primary Care, Fishers Hospital Medical Office Building
  • Schedule may be adjusted based on patient care needs, clinic volume, and organizational priorities.
