WakeMed Physician Practices invites an experienced Ambulatory Care Nurse (RN) to join our WakeMed team. In this ambulatory role, you will coordinate and deliver patient-centered care, collaborating with providers and the care team to manage diverse cases across the outpatient setting.

The ideal candidate holds NC RN licensure, has outpatient nursing experience, and excels in clinical assessment, education, and care coordination to support high-quality, continuous patient care.

Formación

• RN licensure in NC required.
• Experience in outpatient nursing settings preferred.
• Strong assessment and education skills essential.
• Ability to manage multiple priorities in a fast-paced ambulatory environment.

Responsabilidades

• Provide nursing care management and clinical support for ambulatory patients.
• Triage concerns, assess symptoms, and guide care via phone, portal, or in-person.
• Collaborate with physicians and care team to coordinate individualized plans of care.
• Identify high-risk patients and coordinate follow-up, referrals, and resources.
• Educate patients and families about conditions, plans, medications, and preventive care.
• Act as liaison between patients, families, providers, and the care team.
• Promote continuity of care and support optimal outpatient outcomes.
• Maintain accurate documentation and support quality and patient experience initiatives.
