Qualifikationen

• Requires RN licensure in NYS with current registration.
• BSN or equivalent degree in nursing or related field.
• 3–5 years' ambulatory care, managed care, or acute med-surg experience for care coordination.

Aufgaben

• Provide care coordination for assigned patients per policies and program requirements.
• Attend телeconferences and trainings; support program/initiative activities.
• Orient and train new staff; participate in care coordination standards development.
• Act as a clinical role model and ensure culturally competent patient care.
• Monitor activity of staff to ensure policy compliance and timely completion.
• Use Epic for care coordination activities and documentation.
• Engage patients to manage health, medications, and follow-up appointments.
