Job summary

Legacy Community Health in Pasadena, TX is seeking a Clinical Pharmacy Specialist to provide direct patient care, manage chronic disease medications, and collaborate with a multidisciplinary team to optimize therapy.

This role includes patient counseling, medication management visits, precepting students, and contributing to quality improvement initiatives. Requires PharmD (or B.S. with experience), Texas license, ASHP PGY-1; PGY-2 and BCPS/BCACP are preferred.

Qualifications

PharmD or BS with relevant experience and licensure in Texas.
Completion of ASHP PGY-1 Pharmacy Residency is required.
Board-certified or eligible in pharmacotherapy/ambulatory care preferred.
Experience in HIV/geriatrics/pediatrics is desirable.
Ability to manage multiple deadlines and priorities.

Responsibilities

Provide pharmaceutical care through direct patient interaction and collaborative practice.
Conduct chronic disease medication management either face-to-face or virtually.
Obtain and summarize comprehensive medical histories and reconcile meds.
Document encounters in the electronic medical record and monitor outcomes.
Design and monitor therapeutic plans using appropriate medications.
Order labs/tests to evaluate efficacy and safety; report adverse events.
Educate patients, families, and providers about medications.
Precept pharmacy students and residents; participate in team care activities.
Work independently and as part of a collaborative care team.
