Job Description

Clinical Pharmacist

The Clinical Pharmacist provides expert pharmaceutical care services, including:

• Pharmacotherapy consultations
• Disease management support
• Medication reconciliation
• Assessment of polypharmacy issues
• Recommendations to support the patient's longitudinal care plan
• Clinical reviews, including drug utilization review (DUR) for medication orders

In specific departments, this position will also involve analysis of drug utilization data to identify trends and develop strategies to enhance patient outcomes. Additional responsibilities may include committee support for formulary management across the continuum of care and compounding, preparing, labeling, and dispensing medications.

Key Responsibilities

• Provide face-to-face, telephonic, or virtual pharmaceutical care services to patients, promoting safe, effective, and cost-efficient drug therapy recommendations.
• Assist patients and healthcare providers with medication profiles, ensuring alignment with patient formulary coverage to support prescribed care plans.
• Conduct medication monitoring, disease state management counseling, and collaborative drug therapy management.
• Document all patient interactions and interventions in the electronic medical record (EPIC).
• Develop patient-specific educational materials related to disease conditions.

Qualifications

• Active or Temporary State of Nevada Pharmacist license required.
• Certification in AHA Basic Life Support (BLS) is required.
• Preferred experience of two years as a practicing pharmacist, with knowledge in medication therapy management.
