Ambulatory and Community Clinical Pharmacist

We are seeking an experienced and dedicated Ambulatory and Community Clinical Pharmacist to join our healthcare team. This full-time position offers varied shifts with the opportunity to work primarily in an ambulatory setting, focusing on optimizing patient medication therapies and improving health outcomes.

Job Description/General Duties:

• Review and evaluate medication orders for appropriateness and completeness based on patient-specific data including age, height, weight, diagnosis, and laboratory values.
• Communicate with prescribers to provide recommendations and resolve any issues with medication orders.
• Document clinical interventions and resolutions accurately in the patient management system.
• Participate in formulary management as guided by the Pharmacy and Therapeutics Committee.
• Utilize clinical pharmacy skills to provide recommendations to physicians, enhancing patient safety and minimizing risks associated with drug therapy.
• Support decentralized pharmacist activities as appropriate.
• Review and verify medication orders in the medication management information system in a timely and accurate manner.

Minimum Requirements:

• At least 1 year of experience in a retail setting.
• Current pharmacy licensure.
