Qualifications

• Active Ohio Pharmacist license in good standing.
• Minimum one year of inpatient hospital pharmacy experience.
• Ability to work 40 hours per week with varied shifts including weekends/evenings.
• Epic Willow Inpatient system familiarity preferred.

Responsibilities for a PHARM:

• Work 40 hours per week on varied shifts
• Cover weekends and evening shifts as required
• Work typical shift of 12:30-9 PM
• Adapt to variable weekend hours depending on staffing needs
• Maintain an active Ohio Pharmacist license in good standing
• Apply a minimum of one year of inpatient hospital pharmacy experience
• Utilize Epic Willow Inpatient system for pharmacy documentation (preferred)
• Perform pharmacokinetic dosing, specifically vancomycin management
• Manage warfarin therapy for inpatient populations
• Adhere to facility policies and procedures for pharmacy operations
